Please note, you are required to furnish a current immunization record, or the appropriate exemption form, for your child or children at the beginning of each school year.

Please note: If you plan to file a nonmedical exemption form, you must watch the Online Educational Module. At the completion of the module, you will be able to complete an electronic form to obtain a Certificate of nonmedical exemption. You can complete this form for more than one child without having to retake the course.

BCD Preschool immunization reporting
The state of Colorado requires that schools report their immunization and exemption rates annually. During the 2022-2023 school year, our percent immunized rate for MMR was 98.7% and our percent exempted was 1.3%.

BCD Elementary & Middle School immunization reporting
The state of Colorado requires that schools report their immunization and exemption rates annually. During the 2022-2023 school year, our percent immunized rate for MMR was 9.7% and our percent exempted was 3.6%. We did not have any students who were non-compliant.
